How Know Me Support Me Works:

How long does the process of compiling the support plan take?

This process can vary as everyone is different!  We would like this to be an exciting venture for you and will complete the plan as quickly as possible. We can gather information from you initially at your home, online via skype/zoom or phone, and then gradually we would complete the process over a few consultations, either in person or whatever suits you best.  Changes can be made via emails as you think of them.  This is an extensive plan, so it will take some time, but will be a valuable tool for you moving forward.

Can changes be made and how often can this happen?

We will contact you every six months to do updates on your plan.  If you need to make changes in between these times, this can easily be done via email or consultation. 

How do we receive our plan?

Your completed plan will be presented professionally in a folder. You have full control over who sees the plan, and how much of it you wish to share i.e. whatever you feel is relevant. You will also receive a pdf copy to share easily with those that need a digital format.

How much does it cost to compile a support plan?

Initial Support Plan: Please contact us for a quote. We will require a deposit to begin.

Review/Updates: Price on individual updates and reviews will be negotiated based on the number or type of changes required.

Can I claim my support plan under the NDIS?

Your Whole of Life, extensive support plan can be claimed as a support from your Core support funding, or your Capacity building supports if you are self-managed or plan managed. If you are unsure about this process, please contact us to discuss how you can claim our services as part of your plan.

 As yet, we are not an NDIS provider, so if you are agency managed you will not be able to claim our service from your NDIS funding. However, this does not mean you can not access our service, it just means you will pay for it without NDIS funding. You may wish to discuss this with your Local Area Coordinator who may be able to help you access our service.

What information does the support plan cover?

It covers all aspects of your daily life, from morning to night.  It will cover meals, clothes, routines, travel, communication, behaviors and more.  The plans will be individualized to meet each person’s needs.

Will our information remain private?

Absolutely.  Our priority is that your information and support plan remain completely private and confidential as per privacy laws.  You choose who can access your plan and when. 

Will this information be used for other purposes?

No.  This information is yours, and you will be the only person who decides what purpose it is to be used for.
