As a parent of a child living with special needs, I understand that no one knows my son like I do. Being his primary support person, I have, over the years, gathered a substantial amount of information that is required to safely support him in his daily life.
A real concern that I, and I know many others also have is,
‘What will happen to my son or daughter if something happens to me, or to us?
To find support that will meet his extensive needs, if I am unable to care for him, would be extremely challenging without the necessary information relevant to him.
The requirements of individuals living with special needs cannot always be easily passed on. Important information can often be forgotten. Continuously having to repeat information to various support organisations and workers has often left me concerned about the consistency and quality of support.
I decided their needed to be a service available for this information to be compiled for families, to ensure better, more consistent support for the people they love.
Support workers have confirmed that having this information would assist them in better fulfilling their roles.
This is how and why KNOW ME SUPPORT ME was born.
KNOW ME SUPPORT ME is a service to compile a Whole of life, extensive support plans for you or your family member who may need it.
How can it be used?

A parent guardian needs to go into hospital and support is required for a family member.

Parents would like to go away on holidays, but your family member will not be joining you

You your family member goes into supported accommodation for a weekend, a week or longer

Your child is starting school with a new teacher. Instead of taking years to get to know your child, they have the information they need immediately to far better support your child.

You/your family member attends holiday programs or day programs. You then supply just the relevant information to them to ensure appropriate support.

Your support plan can be an extension of your will to ensure your family member is well supported if you pass away.

You have support workers coming to your home who may be unfamiliar with your family member

You are starting therapy programs with new workers
Their are so many ways in which this vital information can be used ensuring peace of mind for you and your loved one.
